I had an eye exam recently but before I went, I checked with my insurance provider and the they said that my exam should be covered 100%. I was only required to pay a $20 co-pay (out of pocket) to the doctor for my visit. However, I ended up paying out $100 total for the visit and I'm not sure of the optometrist's reasoning for this.
