A survey about my boss 51>48

I've been working with a boss for over 6 months who is passive/aggressive towards both the staff and the children in our care. I have tried to report the behaviour to our superior in the hope it changes and we can have a supportive and better centre for all involved but unfortunately she's not much better.
Yesterday I went to work but all it does is continual to upset me and I feel I've done all I can to change things particularly for the children and have got to the point it's time for me to just move on. Today I received a request to fill out an assessment survey on a staff member of our organisation who I don't know and has nothing to do with me (no idea why I received it) but I thought maybe I could use the survey to report my boss.

How worthwhile is it to use this survey to report my boss's behaviour instead

Hex 50.4.5.6>48
I'm wondering if Hex 50 is referring to the fact it's a religious organisation but also it's time for transformation for this centre (or me).

Line 4 certainly suggests things have been turned upside down in a lot of ways for both myself and her or can it mean that by filling out the survey the truth will be out and her dignity will be soiled.

Line 5 Filling out the survey will help things and
Line 6 It will definitely bring a bright new place for staff and children

I'd already made up my mind to leave this job (again) despite it being the only work that has come available for me at this time as it is only 1 x 3 hour shift per week.

I decided to fill out the survey because I had nothing to lose and NOW I feel I've done everything I can to support those children in having a nurturing environment but for me whether I stay or go has no attachment for me and I'll be happy to be gone.

I asked What can I expect personally for me from passing on the information re this boss
Hexagram 26 unchanging - Great Taming

From Hilary - This enables you to go beyond your habitual boundaries and nourish yourself on new experience. You can go where more will be asked of you and your gifts will be of greater service; you can sustain values beyond mere survival. The aim is not to achieve perfect control, staying home and accumulating as an end in itself: whatever you accumulate becomes a springboard to new commitments and explorations. Great Taming cultivates the resources for life on a larger scale.

This past week has brought many insights and inner change for me and I've now signed up to become a Kids Yoga Teacher and study Aruyvedic Transformation which includes a business component.
